Pursuing upstream community-level prevention
The Intersections Initiative brings together coalitions from the seven California communities served by Providence-St. Joseph Health System hospitals with the goal of advancing health equity by addressing the community conditions that shape health, safety, and wellbeing. The seven coalitions are tackling issues as diverse as affordable housing, education, economic and workforce development, immigration, community trauma, and civic engagement. The two-year implementation phase began in December 2018 and lasts through December 2020.
St. Joseph Health Community Partnership Fund (The Fund) and Prevention Institute (PI) have been working together since 2016 to better understand the challenges and opportunities for Providence-St. Joseph Health System hospitals and their partners to intentionally pursue upstream community-level prevention. Through dialogue, discussion, training workshops, and strategy sessions, The Fund and PI engaged community benefit leads, multi-sector partners, and residents in a thoughtful planning period that emphasized the importance of moving upstream from programs and services to policy and systems change. Together, they explored which issues had the most traction in the community, identified strategies for impactful and equitable change, and ensured they had the right partners at the table to take action.
Saint Joseph Health Community Partnership Fund
The Saint Joseph Health Community Partnership Fund makes strategic investments in the health and well-being of communities served by our health system. The Fund works to reduce health disparities and promote equity by investing in systems-level impacts on community health issues, and strengthening partnerships and collaboration. Our investments fall into the following four categories: building capacity and partnerships, promoting justice and community leaders, investing in best practices and new ideas, and caring for the vulnerable. Prevention Institute has served as a thought partner to the St. Joseph Health Community Partnership Fund throughout the formation of the Intersections Initiative. They continue to provide individual technical assistance to all seven Intersections partnerships, as well as facilitate larger site-wide learning opportunities around specific topical areas. Prevention Institute (PI) is a national nonprofit with offices in Oakland, Los Angeles, Houston, and Washington, D.C. Their mission is to build prevention and health equity into key policies and actions at the federal, state, local, and organizational level to ensure that the places where all people live, work, play and learn foster health, safety and wellbeing. Since 1997, they have partnered with communities, local government entities, foundations, multiple sectors, and public health agencies to bring cutting-edge research, practice, strategy, and analysis to the pressing health and safety concerns of the day. They have applied their approach to injury and violence prevention, healthy eating and active living, land use, health systems transformation, and mental health and wellbeing, among other issues.
